Please review and bring these Intake Forms in with you for your first session.  The forms may be opened by clicking on the link, then click on "save" and finally, click on "open".  If you have access to a fax machine, you may fax them prior to your first appointment to 916-614-9201.  The HIPPA Privacy Practices form is for your information only and you don't need to bring it in with you.  We do, however, need the Acknowledgement of Receipt of HIPPA Privacy Practices.
